Summary:
Kai Shin Nurse Coordinator provides comprehensive nursing care services utilizing various skills including assessment, planning, intervention, and evaluation.
Additionally, handles all administrative duties to include reviewing intake packets, assist with UA's, appointments, staff clients, and update treatment team on clients status.
Kai Shin Nurse Coordinator Purpose:
Responsible will be responsible for day-to-day administrative support and client services for NSR programs and will be the liaison between KSC and NSR, occasional travel to other sites required, mileage reimbursed.
We have locations in Chaska, Eden Prairie, & Otsego.
Essential duties and
Responsibilities:
1.
Create and update charts in Practice Fusion (PF) for new intakes a.
Ensure the packets are completed and signed b.
Review the intake schedule and determine needs of the client 2.
Helping with UA by verifying lables, lab req form, and prep for shipment 3.
Help clients prepare for appointments in designated areas a.
Provide a schedule for NSR staff a day prior b.
Collect vitals and update med list c.
Log onto doxy for teleahealh appointments and notify the provider 4.
Send out AVSs to nursing and/or appropriate staff a.
Verify script are sent b.
Scheduling follow-up appointments c.
Ensure appointments are in PF and Procentive 5.
Consistent communication with treatment team, admissions, and Kai Shin a.
facilitation of referrals to substance use disorder/mental health servicas as indicated by a client's medical provider, comprehensive assessment, or treatment plan b.
assist with treatment coordination 6.
Assist with med audits a.
Receives info from nursing/staff a list of med refills b.
Verifies in PF the last refills and if they are needed c.
Calls pharmacy if it has been received by facility d.
Messages providers in PF to request refills 7.
Demonstrate cultural sensitivity and competence in delivering care to clients from diverse backgrounds.

Collaborate with physicians, LPNs, and other members of the treatment team when making decisions about patient's medications and additional nursing care 9.
Demonstrates an ability to intervene appropriately with patients who are in crisis or in need of a higher level of medical care 10.
Assist in keeping medical supplies well stocked and up to date 11.
Administration of medication or assistance with self-medication 12.
Assist in monitoring client's physical health and coordination of care for areas of need 13.
Assist with clinic flow for in house providers and Tele-Psych visits 14.
Assist with aftercare services 15.
All other duties as assigned Kai Shin Nurse Coordinator Continuing Education and Training Stay current regarding professional research, techniques, etc.
Complete 245G and all trainings required by NSR staff Attend in-house training and staff meetings.

None Kai Shin Nurse Coordinator Minimum
Qualifications:
18 years of age High school diploma or eqivalent has successfully completed 30 hours of classroom instruction on treatment coordination for an individual with substance use disorder; has either:
(i) a bachelor's degree in one of the behavioral sciences or related fields; or (ii) current certification as an alcohol and drug counselor, level I, by the Upper Midwest Indian Council on Addictive Disorders; and o has at least 2,000 hours of supervised experience working with individuals with substance use disorder.
A treatment coordinator must receive at least one hour of supervision regarding individual service delivery from an alcohol and drug counselor, or a mental health professional who has substance use treatment and assessments within the scope of their practice, on a monthly basis.
Must be free of chemical use problems for at least the one year immediately preceding hire.
Ability to model a lifestyle that is in compliance with NSR Code of Conduct Good driving record, valid Minnesota Driver's License, and current insurance.
Ability to successfully complete MN Dept of Human Services background study.
Comply with 245G and Program Policies & Procedures and MN Statute 148F.
